Reporting an Injury

Notify Workplace Health & Safety and your manager immediately when an incident occurs.  All workplace incidents, injuries, near misses, and blood/bodily fluid exposures (BBFE) can be verbally reported.

What to include:

  • When and where the incident occurred
  • Who was involved
  • What happened
  • What controls were implemented

*If an incident involves a patient, also complete a report in the Reporting and Learning System (RLS) on CompassionNet.

Should an incident occur: 

1) Follow first aid or exposure protocols as needed
2) Once it is safe to do so, tell your supervisor about the incident
3) Call and report the incident as soon as possible – before you finish your shift
