Venous Thromboembolism (VTE)

Venous Thromboembolism (VTE) is a thromboembolic event (blood clot) that develops within the venous system and includes both deep vein thrombosis and pulmonary embolism.  VTE prophylaxis is one of the most common complications of hospitalization and the most common preventable cause of hospital death.

VTE Audits

The purpose of the VTE audits is to measure and provide feedback to the sites about how they are doing with meeting the  VTE prophylaxis policy. Measurement is one component of the VTE Required Organizational Practice (ROP) for accreditation requirements.

Audits are conducted quarterly and our goal is to do 10 - 20 charts per site for rural sites and 20 charts per unit in the urban sites quarterly. The Crede Technologies Audit Tool will be used for VTE Prophylaxis audits to assist with the coordination of measurement and reporting. The audit results are available for viewing on Tableau

Audits are faxed or submitted online to Crede Technologies by the last Monday of each quarter and the data is available for viewing approximately the 15th of the following month. Submitting completed audit forms to Crede Technologies by the deadline is important for auditors to be able to view their audit results the following month. Not submitting on time can result in delayed viewing of audit data till the next refresh of data (15th of the following month).
