Falls Risk Management

Thanks to all of the hard work from the working groups and program areas we are now in a position to begin using the assessment tools and processes created in the following areas Covenant wide:

  • Emergency Department (urban & rural)
  • Inpatient Adult
  • Continuing Care (long-term care & supportive living)  

Pediatric and Ambulatory tools and processes are currently being developed.

CLiC education module for all frontline staff will be available shortly.

Audit tool for Falls Risk Management is available in the AHS Crede software portal.
DDS is in the process of creating a Covenant Health Falls dashboard in Tableau for Covenant Health Falls audit data. More information to come in the very near future.

If you wish to be set up in the AHS audit/Crede system please contact Jessica Lehman at This tool can be used to assist programs in meeting the compliance with the Falls ROPS for evaluation and measurement.
