The Path to Home model has proven successful in setting clinical care and service delivery standards of excellence within program areas such as Medicine, Surgery and Mental Health in both urban and rural acute care settings.  It forms the basis for our Access & Flow Strategy.  

Integrated Access Strategy

The Covenant Health Integrated Access (Patient Access and Flow) strategy can be simply summarized as facilitating timely patient access and flow across the continuum of care while supporting quality patient care delivery and safety. We have aligned our strategy with the following principles employed across Canada and the United States:

  • Improving access and flow is not a project; it is a natural expression of hospital core business and needs to be positioned as such for sustainability
  • Improving access/flow is less tied to physical capacity as it is to process and cultural transformation
  • Keeping accountability, ownership and empowerment close to the front-line staff who have the expertise in making the best decisions about patient flow
  • Transparent and defined performance measures and accountability mechanisms
  • Alignment with other corporate and provincial priorities such as quality and safety and information management such that activities are leveraged for more than one agenda
  • Ensuring patients receive the right care, at the right time, by the right provider
  • Building system wide processes with a viewpoint from a patient centered care lense
  • Platform of inter-professional team collaboration across the continuum of care
  • Meaningful and timely communication between health care providers, site/sector, partners, and with our patients and families
  • Seamless transitions from an acute episode through to community re-entry and support
  • Standardized discharge processes (Path to Home)
  • Responding to the care needs of our growing senior population; innovation in senior health services
  • Integration of specialty programs/services in acute and community care pathways
  • Balancing capacity and demand though a systems approach
  • Supporting our teams to provide the highest quality care and feel fulfilled in their service and engaged in positive change
  • Ensure teams are strategically aligned within the organizational structure
